Advance Care Planning

Join Lutheran Family Services Rocky Mountains to learn of the importance of Advance Care Planning and Advance Directives.

Natalie DeVille, LCSW, an Aging Life Care Manager, will help clarify the steps in advance care planning including assessing your values, understanding how to choose a medical power of attorney, and talking with your loved ones about your wishes. She will also explain the different advance directive forms specific to Colorado, such as Living Will and Medical Power of Attorney, and discuss when and how these documents are used.
